A deposit of $30 will be required to hold the camper spot on your reservation; the remaining balance will need to be paid in full two weeks prior to arrival.

The following information is required for every camper on your reservation or in your group.

Each camper will need to have the following:
-  First and last name
-  Date of birth
-  Gender
-  Home address
-  Parent/guardian's first and last name, email address, and phone number
-  *Persons authorized for camper check out (typically, this is where you would put each youth leader or parent's name who will be accompanying you on the trip). 

*This field is especially important as this ensures that we can identify the adults authorized to pick the campers up from camp at the end of their stay with us. 

The information for the Health forms will include the following: 
- Insurance information
- Immunization records 
- *Medication Order (signed by a doctor)
- Allergy Action plan (if camper has allergies)


*The Medication Form requires a doctor's signature if parents would like their campers to have access to over-the-counter medications during their time here at Snowcamp. Our Health Center will not be allowed to administer these medications without this signed form. 

This information can be uploaded online when a guest receives the Health form link or faxed in to our Health Center.
